Neuropsychological evaluations assess cognitive functioning and its relationship to brain injury, neurologic disease, psychiatric symptoms, developmental history, medications, substance use, and daily functioning. Common referral questions include dementia, mild cognitive impairment, traumatic brain injury, stroke, attention and executive functioning, memory impairment, and diagnostic clarification.

Independent psychological and neuropsychological evaluations for civil, criminal, probate, family, administrative, and disability-related matters. Healthcare
Midwest Evaluations provides independent evaluations for healthcare, disability, and medical-legal referral questions. These may include psychological or neuropsychological IMEs, disability evaluations, treatment planning, bariatric surgery evaluations, spinal cord stimulator evaluations, Intracept evaluations, and other procedure-related or medically relevant psychological assessments.
Educational
Evaluations may address attention, learning, intellectual functioning, neurodevelopmental conditions, accommodations, and functional needs. Adult evaluations are the primary focus, with select adolescent evaluations age 14 and older considered on a case-by-case basis.

Capacity and competency evaluations require careful integration of cognitive functioning, psychiatric status, functional abilities, medical history, collateral information, and the specific legal or administrative standard at issue. Midwest Evaluations primarily specializes in evaluation and consultation. This helps preserve objectivity, especially in forensic and independent evaluation contexts. When appropriate, reports may include treatment recommendations or consultation regarding treatment options. Direct treatment services are not the primary focus of the practice.
Consultations, interviews, and collateral contacts may be completed remotely when appropriate and legally permissible. Psychological and neuropsychological testing generally requires in-person administration.
No. All services are self pay.
Professional time is billed hourly. Travel time and travel-related expenses may apply when services require travel. Fees, retainers, and payment expectations are explained in the service agreement before work begins.
Yes. A retainer is typically required before services begin. The required retainer depends on the nature and scope of the referral and is explained in the Contract for Services and Fee Structure document.
